On July 31, 2014, the Hon. David O. Carter of the United States District Court for the Central District of California approved a class action settlement requiring United to implement specific protocols for allowing consumers to opt out of United's mail order program for HIV/AIDS specialty medications, as well as a procedure for resolving disputes over participation  in the program.  To download a copy of the Settlement Agreement or  the final approval order, click on the documents below.

If you are to receive specialty medications for HIV/AIDS and United is your health care provider, you can opt out of the program using various methods, including asking your specialty pharmacist to do so on your behalf. To opt-out of the mail-order requirement, patients may either download and submit the form available at http://tinyurl.com/puf2epr or call 1-866-803-8570.  If they call, patients are asked to have their membership number and the name and address of the in-network pharmacy they want to designate to obtain their HIV/AIDS medications.  United is also setting up an on-line method through the United website to opt out of the program.  As soon as that method is set up, we will provide a link here. 

The deadline to submit claims has passed and the claims are being processed.  Final determination of claims should be completed in July 2015 and any checks sent out shortly thereafter.
